According to the Laws of the State of Florida, when applying for a permit, the contractor must have the qualifier for the company sign our building permit application form and owner’s affidavit form. In addition, bring the original State Certification Card to the Building Department when submitting the application.
If you are a State Registered contractor, the Building Division requires that proof of your State Registration Card and Dade County Certificate of Competency be shown at the time of application. The qualifier must also sign the building application form.
If you are not required to hold a State Certification or a Registration Card, the Building Department will accept a Dade County Competency Card. The qualifier must also sign the building application form. If the qualifier is unable to sign the building application form in person, the Building Department will accept a notarized application that shows the qualifier’s signature and appropriate license number. We do not accept Blanket Letters of Authorization (one per application is allowed).
